Program requirements
Vanguard encourages you to play an active role in your health. Completing your biometric screening and the annual health assessment are important first steps in understanding your health status. These two components, along with reporting your tobacco status, are required to earn incentives under the program.

Requirements
To qualify for incentives under the Health Smart Rewards wellness program, you and/or your covered spouse or domestic partner must complete the following by September 30:
- Biometric screening (including signed authorization); see convenient screening options below.
- Annual health assessment on Aetna's Health℠ dashboard, including a signed or electronic authorization.
- Tobacco status on Aetna's Health dashboard.
You will not receive your Health Smart Rewards dollars until you complete all three program requirements (including the required authorizations). The sooner you complete these three requirements, the sooner you'll begin earning your reward dollars. Please note that you will not see any earned rewards in the Rewards Center on Aetna's Health dashboard until all three requirements are complete.
Contact Crew Central™ at 844-VG1-CREW or Ext. 1CREW if it is medically inadvisable or unreasonably difficult for you to complete one or more of the three requirements above, as Vanguard may waive one or more of the requirements and give you the opportunity to earn Health Smart Rewards dollars by completing qualifying healthy activities.
Why the screening and health assessment are important
The biometric screening gives an accurate measurement of your health status regarding obesity, blood pressure, blood glucose, and cholesterol. It's important to have an annual screening to find out whether you're at risk for any serious health conditions.
When your screening results indicate you're at risk across multiple measures, it places you at an increased risk of heart disease, diabetes, stroke, and other serious health concerns. While the biometric screening gives a snapshot of your current health, the health assessment gives a glimpse into your future health. Both can put you on the right path for today as well as tomorrow.
Rewards
You and your covered spouse or domestic partner can earn up to a maximum of $500 each in Health Smart Rewards. You can earn the following by just completing the program requirements:
- $50 for completing your health assessment.
- $50 for completing your biometric screening.
- $100 for self-reporting on Aetna's Health dashboard that you have been tobacco-free for at least six months.
- Up to $400 for meeting or improving biometric screening targets.
If your biometric screening results and tobacco status do not earn you the maximum reward, other qualifying healthy activities (including programs to help you quit tobacco) are available to help you earn any remaining rewards and move the dial on your biometric results.
Screening options
Vanguard has partnered with CrewCare and Quest to provide several convenient ways to get your biometric screening.

Visit Aetna's website, navigate to the Rewards Center and click the Get a Biometric Screening card. Follow the instructions to schedule an appointment or simply find a location near you if no appointments are necessary. Be sure to print a health screening voucher if required by the provider to ensure you receive credit. |
Wellness notice and authorization
Participation in the Health Smart Rewards wellness program is voluntary.
This wellness notice outlines the health information that will be collected as part of the wellness program, how it may be used, who will receive it, and how your information will be kept confidential.
*Please note that before you or your covered spouse/domestic partner complete your biometric screening, you must agree to have your biometric screening results used and disclosed by signing the authorization on the biometric screening voucher or health screening form (whichever is applicable). In addition, before you and your covered spouse/domestic partner complete your health assessment, you must agree to have your health assessment used and disclosed by providing authorization electronically when completing the online health assessment or in writing when completing a paper health assessment.
Privacy
At Vanguard, we respect your data and privacy as much as we do that of our investors. Our wellness partner, Aetna, provides Vanguard with anonymous results in aggregate form, which we use to ensure we're offering programs that best meet your health and wellness needs. Only you see your results.
Note: If you or your covered spouse or domestic partner cannot meet a biometric target or participate in a healthy activity, you may be able to earn the same reward by different means. Contact Crew Central, and a benefits specialist will work with you (and, if you wish, your doctor) to find other reasonable alternatives that are right for you in light of your health status, and also to determine whether a waiver may be appropriate in your situation.
