Making an appointment
It's easy to make an appointment at CrewCare by phone (Ext. 1WELL or 610-669-WELL) or by using My Premise Health, our secure patient portal. We recently moved to an enhanced patient portal, designed to improve your experience and electronically capture your medical history. Due to this change, all crew will need to register for the new secure patient portal.
Select one of these three simple ways to register your account:
- Visit or call CrewCare to receive a personalized registration invitation via email or text.
- Complete registration at the time of your next appointment, arriving 15 minutes earlier to do so.
- Email firstname.lastname@example.org and provide a phone number for a return call.
When registering you will need to enter your date of birth and create a username and password. You will also be prompted to enter a preferred email address and mobile phone number for notifications. Note: when making or attending your first appointment, you will be required to update any annual privacy and consent forms and answer any relevant medical and social history questions on My Premise Health.
Once you’ve registered, use the My Premise Health link to log into the secure patient portal to make an appointment and the many more conveniences listed below. If you have questions about logging on to the portal or need assistance using the secure site, call CrewCare at Ext. 1WELL (19355).
My Premise Health conveniences
- Schedule appointments online by choosing the time, location, and provider.
- Request medication refills for prescriptions written by your CrewCare provider.
- Communicate directly with your CrewCare provider through secure, private messaging.
- Submit and view patient intake forms.
- View your health record.
- Review lab results.
- Pay your bill.
- View your immunization history and health reminders.

Note: CrewCare will still see you for your appointment if you are running late for your scheduled time, but please make sure to notify CrewCare if you will be late.
